Typhoid has emerged as the most widely confirmed illness during India's monsoon season, with recent data revealing that approximately one in four tests (25.5%) return positive. This significant positivity rate positions typhoid well above other common monsoon fevers such as dengue (8.5%), chikungunya (1.8%), and malaria (1.4%), indicating a pressing public health concern beyond typical mosquito-borne diseases.
Rising Fever Risk Across India
The analysis from PharmEasy's Fever Watch, tracking health trends across 209 cities, shows an overall increase in monsoon-related health risks. The national fever risk score escalated from 47 at the season's start to 61 by mid-August. This rise correlates with an increase in the national weather and breeding signal and a substantial jump in rainfall across the country.
While positivity rates for dengue, chikungunya, and malaria have seen a decline compared to the previous year, typhoid's prevalence has remained consistently high. This makes it the most prominent confirmed fever signal in the current dataset, underscoring the shift in the monsoon fever landscape.
Typhoid: A Waterborne Challenge
Unlike mosquito-borne illnesses, typhoid is primarily associated with the consumption of contaminated food and water. This highlights the critical importance of ensuring drinking water safety, maintaining proper food hygiene, and improving sanitation practices, especially during the heavy rainfall months when contamination risks are heightened.
"Every monsoon, fever is often dismissed as 'viral' or immediately associated with dengue. But our data shows that the picture is more complex," stated Gaurav Verma, Chief Business Officer at PharmEasy, emphasizing the need for a broader understanding of monsoon illnesses.
High-Risk Cities Identified
Typhoid is a primary driver of risk in many regions, propelling seven out of the ten cities classified as "HIGH risk" by Fever Watch. While Panaji recorded the highest overall risk score at 94, largely due to dengue, cities like Bokaro and Gwalior followed closely with scores of 79 and 78 respectively, both significantly influenced by typhoid cases. Gwalior and Amritsar, for instance, recorded a laboratory signal of 100 for typhoid.
Cities including Karimnagar and Ramagundam in Telangana, Chandrapur in Maharashtra, and Jaipur in Rajasthan have also experienced particularly sharp increases in their overall fever risk scores.

PharmEasy cautions that some typhoid tests can yield false-positive results. Therefore, accurate diagnosis and appropriate treatment should always be guided by a qualified medical doctor, rather than relying on self-diagnosis. Historical patterns suggest that overall fever risk typically remains elevated from August into the later monsoon months, with diseases like dengue often peaking in late October.
