India has laid robust groundwork for integrating artificial intelligence into its healthcare sector. However, a new knowledge paper reveals that AI-enabled medical technologies are struggling to transition from successful pilot programs to widespread clinical application. The report identifies this scaling challenge, rather than proving AI's efficacy, as the primary hurdle for India's AI healthcare ambitions.
Report Highlights MedTech as Key Opportunity
Union Health Minister J. P. Nadda unveiled the paper, titled "AI in MedTech: Revolutionising Healthcare Through Artificial Intelligence," at the ninth edition of India Medical Device 2026 in New Delhi. Jointly developed by Praxis Global Alliance and FICCI, the report pinpoints MedTech as India's most immediate and scalable opportunity for AI adoption.
Medical devices, operating at the point of care, generate crucial clinical data that AI can enhance to improve diagnosis, clinical decision-making, and workflow efficiency. This is particularly vital for India, a nation facing shortages not only in medical devices but also in specialist medical expertise. By embedding AI directly into these devices, high-quality screening and decision-making could become accessible closer to patients, without needing a proportional expansion of traditional healthcare infrastructure. AI-enabled diagnostics are projected to be the first major application to achieve scale, leveraging structured data and standardized workflows.
Three Major Barriers to Scaling AI in Healthcare
Despite rapid technological advancements and increasing acceptance among clinicians, few AI-enabled MedTech solutions have moved beyond initial pilots. The report identifies three significant barriers:
- Fragmented Health Data and Limited Evidence: Inconsistent health data, poor interoperability between systems, and insufficient real-world evidence complicate clinical validation, regulatory approval, and post-market monitoring of AI solutions.
- Evolving Regulation: Adaptive AI systems require predictable, lifecycle-based regulation that can accommodate continuous software updates and ongoing performance monitoring, a framework still developing in India.
- Inadequate Reimbursement and Procurement Mechanisms: Current payment and procurement systems often fail to recognize the inherent value created by AI-enabled technologies. This lack of recognition dampens incentives for healthcare providers to adopt them and for companies to continue innovating.
India's Strong Foundational Initiatives
The paper acknowledges that India has already established several key foundations for AI-led healthcare. These include initiatives like the Ayushman Bharat Digital Mission, IndiaAI Mission, the Strategy for AI in Healthcare in India, the Benchmarking Open Data Platform for Health AI, and the National Medical Devices Policy, 2023.
However, these foundational efforts must be synergistically linked with stronger evidence generation, predictable regulatory frameworks, and effective reimbursement and procurement mechanisms. The report calls for coordinated action from the government, regulatory bodies, healthcare providers, payers, industry, and academia to achieve this integration. India possesses a unique advantage with its robust software capabilities, digital public infrastructure, clinical diversity, and growing MedTech manufacturing base, positioning it not just to adopt AI, but to emerge as a global leader in the responsible development and deployment of AI-enabled MedTech.
